This form must be completed by anyone who is entering the show ground. Please state which bay you are and your member name. Ensuring all names are present. If your name is not on the form you will not be allowed in.

Please be aware that this information will be cross referenced with the entry forms submitted. Anyone found with more people than is allowed will not be admitted.
